THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective Date: September 7, 2026
Introduction
At Prairie Star Physical Therapy, we are dedicated to protecting the privacy, confidentiality, and security of your Protected Health Information (PHI). As a mobile physical therapy practice currently serving Johnson County, Kansas and expanding into surrounding areas soon, we create, receive, and maintain health records to provide personalized care directly at your location.
We are legally required by federal law (including HIPAA) and applicable Kansas state laws to maintain the privacy of your health information, to provide you with this detailed Notice of Privacy Practices, and to abide by the terms of this notice currently in effect.
Uses and Disclosures of Health Information
We may use and disclose your Protected Health Information (PHI) for treatment, payment, and healthcare operations without requiring your specific written authorization:
1. Treatment
We use your PHI to provide, coordinate, and manage your physical therapy care. This includes conducting initial evaluations, developing plans of care, delivering specialized therapeutic techniques (such as manual therapy, dry needling, Blood Flow Restriction therapy, kinesiotaping, and functional exercise), and communicating with other healthcare providers involved in your treatment.
2. Payment
We use and disclose your PHI to bill and collect payment for services rendered. This includes processing claims through Medicare Part B, submitting invoices to third-party insurers, or processing self-pay/private-pay statements and Good Faith Estimates.
3. Health Care Operations
We may use and disclose your PHI to support the operational management of Prairie Star Physical Therapy. This includes quality assurance reviews, compliance evaluations, software maintenance, administrative audits, and business planning.
Permitted & Required Disclosures Without Authorization
We may also disclose your PHI without prior authorization in specific legal scenarios:
Required by Law: When mandated by federal, state, or local statutes.
Public Health & Safety: To report public health risks, disease control, or mandatory reporting of abuse or neglect under Kansas state law.
Judicial & Administrative Proceedings: In response to court orders, legal summons, or valid subpoenas.
Health Oversight: For authorized audits, licensing inspections, or investigations conducted by state or federal regulators.
Your Rights
You have specific rights regarding the health information we maintain about you:
Right to Inspect and Copy: You have the right to inspect and obtain an electronic or paper copy of your clinical and billing records. Requests must be submitted in writing. We will provide access within 30 days. A reasonable cost-based fee may apply for copies or delivery.
Right to Request Amendments: If you believe your health record contains inaccurate or incomplete information, you may submit a written request for an amendment stating the reason for the change. If denied, we will provide a detailed written explanation within 60 days.
Right to an Accounting of Disclosures: You may request an accounting (a list) of certain disclosures of your PHI made by Prairie Star Physical Therapy for purposes other than treatment, payment, healthcare operations, or disclosures previously authorized by you, covering up to six years prior to the request date.
Right to Request Restrictions: You have the right to request restrictions on how we use or disclose your PHI for treatment, payment, or healthcare operations. While we are not required to agree to all requests, we are obligated to honor requests to restrict disclosures to a health plan if the service has been paid out-of-pocket in full.
Right to Request Confidential Communications: You may request that we communicate with you about your healthcare matters using alternative methods or locations (e.g., specific telephone numbers or alternative mailing addresses). We will accommodate all reasonable requests.
Right to a Paper Copy of This Notice: You have the right to obtain a paper copy of this privacy notice at any time upon request, even if you previously agreed to receive it electronically.
